16 plus Accommodation and Support  for care leavers

CM Supported Living provides accommodation and support to young people age 16plus. We transform the lives of looked-after young people with the most complex needs in the UK.

The team delivers personalised support for each young person, treating everyone as an individual with their own background, experience and ambitions. Our accommodation provides a safe and comfortable environment, operating a robust on-call system with 24- hour key worker access. Each young person has their own agreed support plan based on their needs.

Our supported living service has the resources to support and encourage academic development. We promote positive connections with local Further Education Colleges, employment and training providers.

We support and enable young people to get back to education, training and move on to secure employment.

Support and care is tailored to individual needs through a structured care and support plan, using the Life Star model. It covers ten areas of support:

  • Communicating
  • Feeling good
  • People you know
  • Mental health and other conditions

 

Living facilities are comfortable, decorated to a high standard and designed to promote increased independence. All residents are able to choose their own decor and have their own private bedrooms with shared bathrooms. There will also be shared living areas and hallways.

All homes have large kitchens, living rooms, a fully equipped kitchen/ diner

 

 

Learning Disability

CM Supported Living provides supported housing services, specialising in mental health, homelessness, disabilities (including learning, physical, acquired brain injury and complex needs) and young people services. We focus on person centred packages of care that maximise independence, autonomy and quality of life. We also offer Supportive Living services and community support packages of care across the wider group. We support people with substance misuse issues, mental health conditions, learning disability and offending behaviour.

We provide expert tailored support and care for people aged 18 or over with mild, moderate or severe disabilities, including:

  • Autism
  • Sensory disabilities
  • Physical disabilities
  • Acquired brain injury
  • Associated mental health needs
  • Behavioural challenges

Our services meet the standards set by the Care Quality Commission and the Social Services Inspectorate Wales. They monitor, inspect and regulate our services to ensure that they meet the fundamental standards of quality and safety. Our care home is carefully designed to offer spacious, safe and homely environments, decorated and maintained to a high standard with a conducive atmosphere.

Our services within Hampshire are designed to give individuals the support that is right for them. We achieve this by tailoring our care package to suit each person’s requirements using the Life Star Model to clearly identify a pathway towards.

Our emphasis is on positive care pathways and rehabilitation enabling individuals to move on from hospital. Careful risk assessment coupled with positive risk taking is very much at the heart of the service we provide.

We actively promote and encourage individuals to re-engage in meaningful occupation including areas such as education, positive social integration and leisure and vocational opportunities. We also encourage and support individuals in community based substance misuse recovery programmes.

All staff are fully trained.

Our approach to delivering the support is always totally person centred and individually designed for the specific needs of person.We can help you to:

  • Manage medication and engage with mental health treatment programmes.
  • Establish and successfully maintain a tenancy.
  • Develop or regain independent living skills e.g. cooking, shopping and household tasks.
  • Identify and apply for suitable move-on accommodation.
  • Build motivation, confidence and self-esteem.
  • Manage personal finances including claiming benefits, budget planning, paying bills and developing money management skills.
  • Access education, training, volunteering or employment.
  • Live a full and active life in the local community.

Emotional and behavioural support can be provided by our positive behaviour support team who are very much ‘hands on’ and accessible to the people we support. We also work closely with the local Community Mental Health Teams.

Domiciliary Care

We offer a unique service of personalised care visits from check-in visits to full live-in support.

We, at CM Supported Living Services, specialise in the provision of high-quality homecare and support to keep you safe and comfortable in your own home. We cater for all age groups (younger adults to elderly individuals) with a multitude of needs ranging from physical support to mental Health and Learning Disabilities.

We want to listen to your wishes, preferences and desires about the support you require and how you want it to be given to you. Our staff members want to support you to maintain your chosen lifestyle. We believe that each customer is unique, with their own individual preferences and needs. So, we keep you in control and provide you with the homecare and support that you want, where and when you want it. Our view is that it is your life and your care, so it must be your way.

We offer flexible care services for you to choose from. These services could be anything from a 30-minute visit to live-in-care and ongoing 24-hour staff presence support with a core staff team which may be complimented with additional one to one or two to one support as and when required. Some individuals may want short term help, following an illness or because their usual carer is having a break whilst other individuals may want long term homecare and support/ 24-hour specialist staff support.
